TERAPI ORIENTASI REALITAS PADA PASIEN DENGAN WAHAM KEBESARAN DI KLINIK REHABILITASI JIWA: CLINICAL CASE REPORT
Abstract
Terapi orientasi realitas berperan dalam mendorong penderita waham kembali pada kenyataan dengan tidak membenarkan ataupun menentang waham yang dimilikinya. Pasien berusia 41 tahun dengan diagnosa medis bipolar disorder di Klinik Rehabilitasi Jiwa daerah Kabupaten Garut, diketahui memiliki waham kebesaran yang ditandai dengan keyakinan bahwa dirinya merupakan seorang keturunan keraton yang memiliki kerabat dari Timur Tengah. Penelitian ini bertujuan untuk menggambarkan pemberian asuhan keperawatan jiwa melalui terapi orientasi realitas pada pasien bipolar disorder dengan waham kebesaran. Penelitian ini menggunakan metode clinical case report. Intervensi terapi orientasi realitas dilaksanakan selama lima hari implementasi dan satu hari evaluasi dengan durasi intervensi 1 x 15 – 30 menit per hari. Hasil evaluasi menunjukkan pasien telah menyadari gangguan proses pikir dalam dirinya, namun menilik dari respon agresivitas dan intensitas waham pada pasien yang tidak stabil selama pelaksanaan implementasi, maka masalah pada pasien belum dapat dinyatakan selesai sepenuhnya. Diharapkan, terapi orientasi realitas dapat dilanjutkan oleh perawat klinik disertai dengan optimalisasi pemberian dukungan keluarga dan tindakan perawatan kolaboratif
